・Public Assistance Certificate
(Medical expenses for the Aged, Specified Disease, Severely Disabled, Single Parent, Infant, etc.).
・Certificate limiting high cost medical expenses
・Certificate reducing food costs (If applicable)
What to present at the ward
・Patient ID Card
・Hospitalization Pledge Form
・Certificate of Release from another hospital (Declaration of Hospitalization History)
Hospitalization fees are determined utilizing a system of calculation (DPC) where a fixed daily amount for medical costs is decided by the name of the disease and the type of surgical operation or procedure.
・Your bill will be prepared on the 15th of every month. The bill will be delivered to your
hospital room if you are still admitted to the hospital. The bill can be paid at the automated payment machine on the first floor or at the cashier’s counter also on the first floor.
Payment can be made during holidays and during evening hours.
・The bill upon discharge will be prepared at the reception counter on the first floor. Please pay the bill at the
automated payment machine on the first floor or at the cashier’s counter also on the first floor.
・Based upon the amount of the hospitalization expenses, you may be eligible for support for the high medical
costs. For more information please consult with the Medical Welfare Counseling Office or ask at the reception counter on the first floor.
・Payment by credit card is also possible.
